Widget needed: pet insurance cost calculator for an insurer's website

We run the online presence for a pet insurance company and want an interactive calculator on the site so visitors get an instant, ballpark price for a policy before they ever contact an agent. The idea is simple: the owner picks the type of pet (dog, cat, ideally other companion animals too), enters an approximate age, and ticks the risks they want covered — accidental injury, illness and vet treatment, and third-party liability if the pet harms a person or someone else's property. The widget then shows an estimated monthly or annual premium based on those inputs.

What matters most is that the pricing logic stays easy for our own team to adjust later — age brackets and risk coefficients get revised periodically, and we'd rather not call a developer each time. Configurable rate tables, editable without touching the widget's code, would be ideal. The interface itself should be lightweight, mobile-friendly, match our site styling, and recalculate the price live as fields change, without a separate submit button.

We're open to your suggestions on implementation — a standalone widget dropped into different pages, or something tied to our content system. Please share examples of similar calculators you've built.
