Create a feedback form

A feedback form is needed with the ability to dynamically change parameters in it, namely, to change the list of ordered services. For example, a person is in category A, clicks the button "Order" and he has a pop-up form showing the application form, and it shows all products from category A, if a person presses the button from category B, then he must have in the form of all goods from category B. Products in the form must be automatically downloaded from our database.
