There's a server on Yii, and it's like a Google form, we compose a survey and we do it. There are several different types of questions in the survey. There are sub-questions (i.e., If we choose this answer, then immediately a sub-question falls out.
The survey is compiled in the admin, there is also indicated the type of question and dependence on subquestions.
Required:
1) Make the division into questions into pages (so that not one big curtain was all, and after the Nth question was the button "Next" and we moved to the next page).
(2) A new type of question is not, more precisely, a question, but simply a text with a reference. The link opens Doc document with pre-filled fields Surname, Name, Patrimony, year of birth and several more. Such links should open the Template of the contract and the Consent to the processing of personal data. So we can continue to celebrate. (In documents, values can be inserted through references.)
(3) Possibility of adding autocomplete. I mean. When answering one question, the answer to it was copied into another question (on another page, just to have the previously filled value before your eyes)
