You need to create an application for android.
In general, the application should work as follows:
1. The user opens the application. A form to fill in appears before him. The form contains input fields (e.g., name), drop-down list (e.g., list of models) and yes/no toggles with text.
2. After the user enters all the necessary data, he presses a button, for example, "Send" on the phone creates a PDF file and sends to you by email.
3. The user does not save any files on the phone, he just sees a message, for example, "Your application is sent, blah blah blah" and can close the application.
