Requires software for the interviewer

Up to now, our interviewers walk through the streets and apartments, interviewing the population using paper questionnaires, after which the information is “stuffed” in the excel table in manual.
We have now decided to consider the possibility of interviewers using tablet computers, for example, on an android.
So. Requires a program-designer of forms for PC (to create an electronic version of a paper questionnaire) and a software shell for Android, which will work with the questionnaires downloaded from the PC (allowing the interviewer to fill them in an unlimited number of times) and forming a matrix of answers in excel format.
The requirement for the user part - to design forms should be easy for the "experienced" PC user. Work with the tablet should be as simple and intuitive as possible (interviewers at the age of 50+ work).
The project starts in January-February 2016. I am currently making a budget for it, so I am ready to consider all the offers on price and quality.
