Formation of a text template from a web form

There is a third-party service, the data in which is entered through the web form and stored there in the database service.

You need a solution (simple) that would form a text template (word, rtf) from the form data and return the user to download.
I will provide the text of the template, you need to make at least 1n example.
Data from a third-party service, after filling in the web form, can be transmitted to POST.
